Home has always been the gathering place for people who care about each other. It's where memories are made and families experience each stage of life together. Home is truly where the heart is, and it's where many older adults want to live out their lives. At the core of the Northland PACE philosophy is the goal of assisting individuals who are at risk for nursing home care to remain living safely in their homes and communities for as long as possible. Northland-PACEThe health needs of each individual may vary, but regardless of their medical needs, Northland PACE plans and coordinates a wide range of health-care and in-home services. A team of professionals called the Inter-Disciplinary Team (IDT) or Care Team, works with seniors and their families to make the best possible health decisions. Services include health-care appointments, which may include scheduling, escorting, and even transportation when necessary, to help participants remain as healthy and independent as possible. The program may also include, upon approval by our IDT Care Team, medications, eye glasses, hearing aids, durable medical equipment, various therapies, and other assistance that PACE doctors and providers may prescribe. Additional in-home services may include personal care such as bathing, dressing, light housekeeping, meals and nutritional counseling.
